By checking this box, I confirm that I have read, understand, and agree to the following policies and fees: Unless otherwise agreed upon, student is still responsible to pay the full session charge if: (1) The session is cancelled without 24 hours notice, (2) The student is a no-show, (3) The student is more than 10 minutes late and therefore considered to be a no-show. All payments submitted later than 1 week after due (first session of each month) will result in a $10/week late fee.
